Tracie Jacobson is a clinical pharmacist at Northwestern Memorial Hospital in Chicago, Illinois. She is the founding member of the Anticoagulation Service which began in 1998. Tracie also precepts pharmacy students and residents from multiple Colleges of Pharmacy and teaches lectures on anticoagulation to the physician assistant program at Northwestern University.
