RNs will learn the importance of care coordination and transition management (CCTM). Attendees will acquire new skills to implement in their practice of coordinating care from a certified clinical navigator in trauma services.

Patient care coordinators help patiients focus in their discharge needs

Great course. I enjoyed the change from 'discharge' to 'transition'. It really refocuses the concept of patients leaving each setting.

I learned a lot from this presentation. It help me recognize how much care coordination bedside nurses do without even realizing it.
