Transitions of Care for the Medical-Surgical Nurse

This town hall session highlights the current state of the transitions of care in the medical-surgical setting. The outcomes from Dr. Naylor’s research are discussed and strategy examples from medical-surgical units around the country that are implementing approaches to address transitions of care are shared with attendees.

  • Identify one of the three new questions on the HCAHPS report that patients will be asked that directly relates to their transition of care.
  • Discuss the approaches that some of the outstanding medical-surgical units around the country are implementing to address the transition of care.
