Keynote Address - Transitional Care

Normal Aging vs. Pathological Aging: Do You Know the Difference?

Older adults often present to the hospital with multiple complex co-morbidities, so medical-surgical nurses have a duty to be knowledgeable in caring for this population. This session discusses how certain conditions are common and preventative and may signal a need for a thorough assessment. The importance of nurses being able to differentiate normal aging from pathological aging is also explored.

Standardized Nursing Assessment of Dyspnea to Reduce Morbidity and Mortality

Dyspnea is an important independent predictor of adverse outcomes in certain populations. This session describes a pilot study on the feasibility of routine dyspnea assessment of all inpatients upon admission. The presenters also explore how dyspnea assessment presents an opportunity to optimize care, improve symptom management, and help predict outcomes.
