One of the big health services that has grown with the COVID-19 pandemic is the use of telehealth. This session discusses care coordination and transition management (CCTM) and highlights utilization of telehealth as a care modality. Discussion includes medical-surgical nurses' knowledge and awareness on CCTM and the role of telehealth.
