pandemic is the use of telehealth. This session discusses care
coordination and transition management (CCTM) and highlights utilization
of telehealth as a care modality. Discussion includes medical-surgical
nurses' knowledge and awareness on CCTM and the role of telehealth.

nursing continuing professional development (NCPD) evaluation. You will
be able to print your NCPD certificate at any time after you complete
the evaluation.
