312A: High Risk, High Care: Proactive Rounding to Safeguard Patients - Proactive rounding on high risk patients is essential to helping to prevent hospital acquired conditions (HACs). This type of rounding helps to identify risks, intervene early and ensure evidence based practices are followed to help prevent HACs. Proactive rounding not only ensures we are preventing harm to the most vulnerable patients but allows an opportunity for real time education with frontline staff.
312B: Mitigating Workplace Violence Using a Three-Phased Approach - Recognizing the heightened risk for healthcare workers, our goal is to proactively support caregivers, patients, and visitors by improving the prevention, response and recovery to workplace violence (WPV) events. Addressing and mitigating WPV is bidirectional involving interaction between the individual employee and organization at large. This project’s three-phased approach to WPV provides leaders with a standardized framework to allow them to tailor their approach based on their team’s needs.
