204A: Pre-Emptive Ethics: Creating Individualized, Adaptive Plans of Care for Patients with Disabilities - This presentation explores the development and implementation of Adaptive Care Planning for patients with intellectual and developmental disabilities (IDD), aimed at addressing the unique challenges these patients face in healthcare settings. It will highlight how interdisciplinary collaboration and ethical reflection can improve care delivery and reduce disparities. An Ethics Consult Service (ECS) has played a pivotal role in creating personalized, evolving care plans informed by input from caregivers and the clinical team, leading to improvements in patient outcomes and staff satisfaction. This initiative demonstrates the importance of equipping healthcare staff with the tools and support needed to provide inclusive, patient-centered care for individuals with IDD.
204B: An Interdisciplinary Approach to Address Social Determinants of Health (SDOH) in a Hospital Setting - This session will explore how an interdisciplinary team addressed social determinants of health (SDOH) to improve patient care and health outcomes. Attendees will learn about the development and implementation of a nursing-led screening tool integrated into the EMR, workflows to connect patients with resources, and the measurable impact these initiatives have made. With a focus on food insecurity, housing instability, transportation needs, utility assistance, and partner violence, this project highlights the critical role nurses play in identifying and addressing health disparities. Learn practical strategies you can take back to your own units to close gaps, spark connections, and deliver care beyond hospital walls.
