Malnutrition plagues many inpatients.   Developing and implementing nutritional plans that span the gap from hospital to home are critical to promoting well-being upon discharge.  Understanding nutrition as a social determinant of health and malnutrition as a risk factor for health care complications, nurses are poised to intervene and provide access to foods that foster health and healing. 

Target Audience: Nurse coordinators, discharge planners, home care nurses, inpatient nurses

Outcomes: 

  • 70% of participants will report that this session provides information that enhances their ability to help patients gain access to food sources upon discharge
