From Hospital to Home: Bridging the Nutritional Gap

Malnutrition plagues many inpatients. Developing and implementing nutritional plans that span the gap from hospital to home are critical to promoting well-being upon discharge. Understanding nutrition as a social determinant of health and malnutrition as a risk factor for health care complications, nurses are poised to intervene and provide access to foods that foster health and healing.
