Description

Pressure injury prevention programs rely on each member of the health care team taking responsibility for protection. From the point of entry to the system to discharge, elevating the importance of skin integrity becomes part of standard workflow. This session provides practical strategies for team implementation.

Thank you for this info. I would like to ask, as a staff nurse, and again our charting, ( I know you get a lot of questions about Braden scale and charting). "when doing skin check and discovering a tear in the gluteal cleft, would you chart it as a "simple wound"/skin tear, or you would rather chart stage 2, and show improvement? a dressing cannot usually be applied in that area, so beside good pericarp, and protective anti moisture cream, and of course turning, is there anything else we should do? Thank you.
