Research has shown that disconnects in communication between health care providers have been linked to adverse patient outcomes and frustration within the health care community. Good communication between nurse, physician, and patient will lead to safe and efficient patient care. Our project has improved patient, provider, and nurse communication and has led to better outcomes.
This educational activity is recognized as care coordination/transition management-focused content by the Medical-Surgical Nursing Certification Board (MSNCB).
