Description

Given the projected growth of the population of older adults with complex care needs, there is a critical need to bridge the chasm between evidence-based practices and current approaches. This session describes the efforts of a multidisciplinary team to advance the science related to transitional care, as well as to translate research findings into the "real world" of clinical practice.

  • Describe key concepts underpinning the Transitional Care Model (TCM) and the key findings from testing the TCM in the real world of clinical practice.
  • Describe impact of body of evidence from testing of TCM on clinical practice and health care policy and the key indicators of success.
